Once a decision has been made to prescribe INVEGA SUSTENNA®
The INVEGA SUSTENNA® Enhanced Patient Support Program is designed to help adult patients with schizophrenia start and stay on therapy. The program helps support your patients in 2 different ways:
The Care Transition Navigator provides support tailored to your patients' needs to help them stay on their prescribed treatment by coordinating:
Led by licensed pharmacy professionals, adherence support is automatically initiated for adult patients first prescribed INVEGA SUSTENNA® in outpatient care or will begin after transitioning to outpatient care, and if their prescription is filled by a participating pharmacy, providing barrier identification and injection coordination for the first 4 months of outpatient care with education on the following:
INVEGA SUSTENNA®
Schizophrenia overview
Managing schizophrenia
Treatment goals discussion
Treatment options: Same medication with fewer doses per year*
*Education on this topic is provided only if the patient opts in.
The INVEGA SUSTENNA® Enhanced Patient Support Program provides your patients with support during the first 4 months of therapy with INVEGA SUSTENNA® so they can feel confident about staying adherent and prepared for the possibility of even fewer doses a year with INVEGA TRINZA® or INVEGA HAFYERA®.
INVEGA SUSTENNA®, INVEGA TRINZA®, and INVEGA HAFYERA® are each dosed differently per their individual Prescribing Information. Please click here to view the Dosing & Administration Guide and follow the Instruction for Use.
The INVEGA SUSTENNA® Enhanced Patient Support Program is limited to education about INVEGA SUSTENNA®, its administration, and/or the condition it treats. It is not intended to provide medical advice, replace a treatment plan you receive from your doctor or nurse, or serve as a reason for you to start or stay on treatment.
Once the decision has been made to prescribe INVEGA SUSTENNA®, just call a participating pharmacy with the patient’s name, prescription, insurance information, and their next care setting, and the pharmacy will start the patient enrollment process
Once a participating pharmacy receives a prescription of INVEGA SUSTENNA® for your patient, a licensed pharmacy professional will reach out to the patient to initiate the INVEGA SUSTENNA® Enhanced Patient Support Program
